3. Refund Policy (IV Therapy, Labs, Hormone Therapy)

Because clinical services involve preparation, supplies, and practitioner time, the following refund policy applies:

IV Therapy Refunds

No refunds are provided once IV supplies are ordered, compounded, or prepared for your appointment.
If you do not show for your appointment, the IV therapy is non-refundable.

Laboratory Testing Refunds

Once lab orders are placed or sent to the lab vendor, no refunds are available.
Prepaid labs are non-transferable to another person.

Hormone Therapy Refunds

Hormone prescriptions and pellet orders cannot be refunded once processed by the pharmacy.
No refunds will be given for services already rendered or medications already dispensed.

4. Cancellation, Late Arrival & No-Show Policy

You may reschedule your appointment if you provide 24-hour notice.
There are no fees for cancellations or late arrivals at this time.
No-shows will not be rescheduled.
(Patients who do not attend their scheduled appointment may be dismissed from future scheduling.)

Longevity Health DNP reserves the right to update this policy at any time.

5. Wellness & Lifestyle Services

Longevity Health DNP provides metabolic health services, hormone optimization, IV therapy, nutritional guidance, and lifestyle recommendations to support general wellness.
